Do I have ADD/ADHD, Please Help!!!?

ok so im 15 and a male. I am very twitchy/jumpy. I never stop moving my feet. I never stop biting my nails. I have troubles sleeping because i just dont feel like it. I can not focus in cl no matter how hard i try. I get tired/fall asleep while reading a book. I am very forgetful even when reminded constantly. I have mood swings (ill be happy one second and pissed off for no reason the next). I day dream alot. I always move no matter what im doing.. I find it hard to write a paper unless it really interests me. I always forget to do my homework because i dont remember even having any. I sometimes forget what i did in during the day. I am always anxious about something. I get very itchy/hot if im not moving. I am a little dyslexic (ill mix up words letters pretty much anything in a sentence). I have a speech problem (mis-pronounce words alot get tounge twisted easily). I am very random. I have a high pitch ringing if there is silence. I am a horrible procrastinator. Im the group jokester
