Need to reschedule a small claims court summons!!!?

I just got in the mail a small claims court and I need to reschedule for at least till middle of May. I am on medical treatments whereby I might need to go in any day at a short notice for surgery. How do I go about extending the hearing dates. I am the defendant. Do I go to court or mail them a letter? I live in NYS.
